Registrar's Office

The mission of the Registrar’s Office is to support the educational mission and core values of Columbia College of Nursing, by serving as the official record keeper for students, faculty, staff, alumni and the general public. The office is dedicated particularly to conducting enrollment services/registration, data management/reporting and maintaining academic records.  The Registrar’s Office has the responsibility to maintain timely and accurate records of the academic progress and accomplishments of its students, while ensuring the integrity, confidentiality, and security of all education records as defined and protected under the Family Educational Rights and Privacy Act (FERPA). 


Course Descriptions

Change of Demographic Information

It is the students responsibility to maintain up-to-date and accurate demographic information. All changes can be submitted electronically via the student information system (SONISWEB). The following demographic changes should be reported as soon as changes occur.

  • Change of address (local and/or permanent)
  • Change in telephone number
  • Change of name (requires official documentation to be submitted to the Registrar’s Office)   


Change of Name

All students at Columbia College of Nursing (CCON) will be granted the opportunity to submit a name change, as needed.  Students will be required to submit the Name Change form and legal evidence indicating the name change to the Registrar’s Office.

Legal documentation includes:

    • Certified copy of a marriage license, court order, or dissolution decree reflecting the new name in full; or
    • Current passport or other government-issued official proof of identity (Wisconsin Driver’s License with photo).  


Address Change

Students can update their biographic/address information in SONIS.


Transcript Requests

  • CCON is able to process official transcripts for graduates from the diploma program prior to 1985.
  • If you graduated after 1985, please contact the Registrar’s Office at either Carroll College or Mount Mary University for your official/unofficial transcript.
  • If you are a current transfer student from another institution, CCON is able to process your official/unofficial transcript requests.

How to request transcripts:

    1. Print out and fill in the Transcript Request Form

    2. Type/print your (the student's) name with your signature and date as required by the Family Rights and Privacy Act (FERPA) of 1974. Requests that do not include a signature will not be processed.

    3. Send completed form along with $6 payment to the Registrar’s Office

Transcript fee:

    • Standard delivery (3-5 days) - $6 each 
    • Urgent delivery (same day delivery/pickup requests)* - $10 each 

Payment accepted by cash, check or credit card, (MasterCard, VISA, American Express and Discover accepted). Make checks payable to CCON.**

*Urgent requests must be placed before noon on the day of pickup/delivery

**NOTE: If there is a hold on your student record, you must resolve that hold before a transcript will be released.

Veterans Benefits

Columbia College of Nursing is proud to support the men and women in uniform who have served our country. It is the responsibility of the student Veteran to be aware of their eligibility for federal and state veteran’s benefits and follow all program requirements. Student Veterans are asked to contact the Registrar’s Office to initiate the procedure to have their Veteran’s Affairs (VA) benefits processed. Students will be required to fill out the Application for Certification of Veterans Affairs Education Benefits before each semester they wish to have VA Education Benefits apply.
Columbia College of Nursing also participates in the Yellow

Ribbon Program. The Yellow Ribbon benefit is on a “first-come, first-serve” basis and will be awarded to the first five eligible students that apply. Columbia College of Nursing agrees to provide Yellow Ribbon Program contributions of $1500 to the first five eligible individuals that apply during the academic year. Students are required to submit a signed, completed Yellow Ribbon Benefit Application to the VA Certifying Official. Student applications will be considered as they are received. When the document is processed, the student will be notified if they are eligible to receive benefits through the Yellow Ribbon Program.

For specific eligibility criteria and clarification of benefit programs, students should refer to the Department of Veterans Affairs website, www.gibill.va.gov or contact their County Veterans Services Officer (CVSO) for assistance in utilizing benefits.
